What is coolsculpting?

Cool sculpting is a unique noninvasive procedure at that reduces fat deposits by freezing the tissue. Here at Salinas Valley plastic surgery, we are the original fat freezers for this area. We have been doing local sculpting longer than anybody in central California. The procedure works because that has been shown to be sensitive to cooling. The machine comes with special applicators that draw the skin and fat in any specific area inside the applicator where two cooling plates freeze it to about 55°F for one hour. When the applicator is removed, the fat is literally crystallized and frozen under the skin. The area is massaged and the fat crystals are broken up. The area looks a little bit red and irritated for about 24-48 hours. Over the course of the next 2-3 months, about 20-25% of the fat in this treated area is gradually reabsorbing by the body. The lipids are absorbed into the bloodstream and they absorb so slowly that it is the about the equivalent of 1-2 French fries a day. Scientific studies have documented with ultrasound measurements that 19-24% of the fat layer is consistently reabsorbed.

How long does a “Coolsculpting” procedure usually take to perform?

The procedure takes one hour to perform for each treatment area. However, unique to our practice here in central California, we have 2 machines that allows us to “Dualsculpt”. Any one undergoing treatment of an area of the body and that is treated on each side of the body, such as the love handles , or the bra fat can have these areas treated simultaneously.

How do you perform a “Coolsculpting”?

The treatment area or areas are marked, and the appropriate size applicator is chosen, a gel pad is placed on the skin, the applicator is applied and the suction is started. Once the skin and fat are drawn into the applicator, the cooling cycle is started. Over the course of the next hour the skin and fat are frozen to 55°. The applicator is then removed and the area is massaged and manipulated to break up the crystallized fat.

Who is a good candidate for a “Coolsculpting”?

The best candidates for cool sculpting or people who have small to medium-sized pockets of fat with overlying good skin quality. In order to work, the skin has to shrink as the fat is reabsorbed after the treatment. Coolsculpting is not a substitute for an area of the body where there is a lot of loose skin. An example of this would be someone who really needs an abdominoplasty in order to remove excess skin along with the excess fat.

What are realistic expectations for a patient having a “Coolsculpting”?

Coolsculpting is not a substitute for someone who really needs surgery to remove a lot of fat or needs excision of excess skin.

What are the common reasons why a patient wants a “Coolsculpting”?

Good candidates for cool sculpting are patients that have unwanted pockets of fat that they can’t get rid of with diet and exercise. It is also for patients who are unwilling or extremely reluctant to have an actual surgical procedure.

How long does it take for a patient to recover from a “Coolsculpting”?

The following effects can occur in the treatment area during and after a CoolSculpting procedure. These effects are temporary and generally resolve within days or weeks.

During a CoolSculpting procedure:

  • Sensations of pulling, tugging, and mild punching.
  • Cold, tingling, stinging, aching, cramping. These sensations quickly subside as the area becomes numb

Immediately after a procedure:

  • Redness and firmness.
  • Transient blanching and/or mild bruising around the edges of the treatment area.
  • Tingling and stinging.

Within one to two weeks after a procedure:

  • redness, bruising, and swelling.
  • Tenderness, cramping, and aching.
  • Itching, skin sensitivity, tingling, and numbness. Numbness can persist up to several weeks after procedure.

Rare Side Effects:

  • Vasovagal symptoms: Dizziness, lightheadedness, nausea, flushing, sweating, or fainting during or immediately after the procedure.
  • Focal neuropathic pain with a typical onset several days after a procedure and resolution within several weeks, easily easily controlled with over-the-counter pain medications or mild non-narcotic prescription medications.

Equipment failure: occasionally, the CoolSculpting equipment may malfunction during your treatment and your treatment will have to be re-scheduled to another day. There won’t be any harm to you or additional charges if this occurs, other than the inconvenience of having to re-schedule.

Matthew L. Romans, MD, FACS

Dr. Romans has extensive training and experience in cosmetic and reconstructive procedures. He has performed thousands of procedures since his arrival to the central coast in 1996. Dr. Romans received his plastic surgery training at Wake Forest University. While there, he worked with world class plastic surgeons that passed on their approach to plastic surgery to him. To this training he added his own artistic ability and the resulting product is a surgeon with a passion for superior results and top-quality medical care.

With his excellent results and sterling reputation, he commonly gets patients from all over California. He has even had patients come from out-of-state and international sites. He keeps up with the latest techniques and skillfully applies them to all his patients. Dr. Romans is an advanced injector for injectable fillers such as Restylane®, Juvéderm® and Botox®. With the use of these injectables he can erase years and give almost anyone a rested and youthful appearance.

He really enjoys cosmetic surgery, but he feels that the continued use of the ability to do complex reconstructive surgery is critical.

Personal Life

Dr. Romans is married and has two children. He has many hobbies including computers, violin playing, running, and reading. His professional sports passions include Baseball, Football, and Hockey. Dr. Romans retired with an honorable discharge as a Major from the United States Army Reserves in August 2002 after serving in a reserve capacity for 12 years.

Jeremy Silk, MD, FACS

Plastic Surgery is a delicate balance of artistry and science, which requires years of training combined with artistic talent. Dr. Jeremy Silk embodies this ideal. Dr. Silk is a double board certified plastic surgeon who brings his unique style to the Central Coast. He is proud to offer state-of-the-art techniques to deliver superior results. He has quickly established himself as a respected name in plastic surgery in the Monterey and Salinas areas.

Dr. Silk was raised near Indianapolis, Indiana. He attended Indiana University on academic scholarship, graduating with a Bachelors of Science in Biology, with Honors. He continued at Indiana University for his medical school training, receiving his MD in 2000. He then completed a five year residency in general surgery at the highly regarded Phoenix Integrated Surgical Residency. His training encompassed the full range of trauma, general, vascular, thoracic, and endocrine surgery. As a part of this comprehensive residency, Dr. Silk received extra training in unusual surgical diseases during special sessions in Anchorage, Alaska. He became certified by the American Board of Surgery in 2005.

Dr. Silk completed his plastic surgery training at the University of Kansas, where his talent for plastic surgery blossomed. He received the full breadth of training from talented surgeons in reconstructive, upper extremity, congenital, and cosmetic plastic surgery. Dr Silk went on to achieve board certification in plastic surgery from the American Board of Plastic Surgery in 2008. He is an active member of the American Society of Plastic Surgeons.

In addition to his academic accomplishments, Dr. Silk has traveled abroad to perform medical missionary work in exotic locations such as Santo Domingo, Dominican Republic, and Eldoret, Kenya. These humanitarian medical missions have provided him with extra expertise to fine-tune his surgical skills. He has presented research at Indiana University, at national meetings for Plastic Surgery residents, and locally for the Monterey County Educational Healthy Consortium.

Dr. Silk is known for a friendly and compassionate style. He takes extra time with each patient to ensure their goals are met. He prides himself on guiding them through the entire process, from first consultation to the final postoperative visit. Dr. Silk constantly reviews the literature, attends meetings, and masters the best new techniques. He strives to ensure that every patient receives the very best results.
